PACE Program

Are you or a loved one worried about a recent diagnosis? Not sure of the next steps?

The PACE Program is funded by the Sydney North Health Network (SNHN). It helps individuals over the age of 55 who are diagnosed with a chronic condition or those at risk of developing one.

Our program provides 1:1 support, essential health information and connections to the right services and groups for healthy ageing and chronic condition management.

How PACE Can Help You

A dedicated PACE Partner will provide ongoing support and guidance to help you:
Navigate your diagnosis and symptoms
Connect with healthcare providers and community groups
Access essential services such as home care, physio, emotional and social support

Our team ensures you have the right support at every step of your journey.

Service Availability:
Monday – Friday 9:00am – 5:00pm

PACE accepts medical, nursing, allied health and self-referrals

Services We Offer

Needs Assessment & Plan

✔ Individual assessment to develop a personalised support plan

Support & Mentoring

✔ Ongoing 1:1 mentoring to help manage your condition effectively

Referrals to Providers & Services

✔ Referrals to healthcare providers, community groups, and social support services

Allied Health Services

6–8 therapy sessions with physiotherapists, occupational therapists, and other specialists, followed by a review

Social & Community Activities

✔ Connecting you with social groups and local programs

Understanding My Aged Care

✔ Help with registering and navigating My Aged Care

Navigating Services & Support

✔ Guidance on available services and how to access them
