KNC Community Nursing team provides nursing support for CHSP, STRC and HCP clients as well as private clients. Our nursing team consists of experienced registered nurses in the following community nursing services:

• Medication management including injections
• Continence management
• Wound management
• Diabetes support
• Respiratory care

While our nurses provide individualised care to ensure that the clients receive the appropriate services for their needs, they also work in partnership with the client's GP, other healthcare professionals and the family to provide person centered care. This may help reduce hospitalisation, provide early interventions for new medical issues and assist with compliance with medical directives.
