Support at Home Program (SaH)

The Support at Home program, introduced in November 2025, replaces the Home Care Package Program and the Short-Term Restorative Care Program. It is designed to make it easier for older Australians to access services, equipment and home modifications that support independent living at home for longer.

Support at Home may be right for you if you need a coordinated approach to care, whether that is help with everyday tasks or more complex and intensive support.

What does Support at Home provide?

Nursing care

Physiotherapy and Occupational Therapy

Podiatry and other allied health services

Independence supports

Help with showering and personal care

Medication assistance

Transport to appointments

Respite care for carers

Everyday living supports

Domestic assistance and cleaning

Gardening and home maintenance

Shopping and meal preparation

Culturally safe care

Assistance to participate in cultural activities and access to Aboriginal and Torres Strait Islander health practitioners and health workers.

Support at Home funding classifications

If you are approved for ongoing Support at Home services, you will be assigned one of eight funding classifications following a My Aged Care assessment. Your classification reflects your individual circumstances, including your functional ability, health needs, risks and overall care complexity.

Funding classifications are numbered from 1 to 8, with higher classifications providing access to greater levels of funding to support more complex care needs.

Ongoing care under Support at Home

Your funding classification determines the level of support you receive and includes funding to help manage your care.

People with lower needs may receive help with everyday tasks such as cleaning, meals and transport. Those with more complex or higher needs may receive clinical care such as nursing or in-home physiotherapy. As needs increase, higher classifications allow for more frequent and coordinated support, helping you remain at home safely.

Short-term Support at Home pathways

Restorative Care Pathway 

This short-term pathway focuses on helping you regain strength, mobility and independence. It provides coordinated allied health and/or nursing services and aims to reduce the need for long-term care while helping you return to the activities you enjoy.

Assistive Technology and Home Modifications (AT-HM) scheme 

The AT-HM scheme provides upfront funding for assistive technology, equipment and home modifications. This may include mobility aids, safety equipment or changes to your home to improve accessibility and reduce the risk of falls.

End-of-Life Pathway 

The End-of-Life Pathway provides additional support to help you remain at home during the last three months of life. You can be referred for a high-priority assessment, even if you are not already receiving Support at Home services.

How to access Support at Home

To access Support at Home services, you will need an assessment through My Aged Care. You can contact My Aged Care on 1800 200 422 or contact KNC on 02 9988 4966 and one of our friendly and knowledgeable staff can guide you through the My Aged Care process and next steps.

Step 2: Assessment and funding options

We will assist you to navigate your My Aged Care assessment and help you understand the funding and support options available to you under the Support at Home program.

Step 3: Design your care plan

Once your supports are approved, our team will work closely with you to design a care plan that reflects your goals, preferences, lifestyle and changing needs.

Step 4: Receive services

Once you are happy with your care plan and have signed your service agreement, we will book your services with our KNC team and preferred providers.
