Home Care Package (HCP)

Home Care Packages are a government subsidised program designed to support older Australians to live in their own home.

There are a broad range of services you can access with your Home Care Package to ensure you can live at home safely, whilst both your clinical and wellbeing needs are met.

How to access Home Care Package (HCP)

To be assessed for a Home Care Package please contact My Aged Care for an assessment on 1800 200 422 or contact KNC on 02 9988 4966 and one of our passionate staff can guide you through the My Aged Care Process.

There are four different levels of funding with Level 1 being basic care needs, and Level 4 catering for people with higher or more complex care needs.

At KNC we offer all 4 levels of care and you will be assigned an experienced and compassionate team member to support you on your Home Care Package journey.

Once we get to know you and understand what’s important to you we’ll provide everything you need to know and more

Step 2: Review funding options

We can help you access funding options including the Commonwealth Home Support Program (CHSP) or a Home Care Package (HCP) via My Aged Care.

Step 3: Design your care plan

We’ll guide you through it all. Once we have your funding option decided, our team will work with you to create a care plan that matches your lifestyle, interests and needs.

Step 4: Receive services

Once you are happy with your care plan and have signed your service agreement, we will book your services with our KNC team and preferred providers.
