Commonwealth Home Support Program (CHSP)

The Commonwealth Home Support Program (CHSP) supports older individuals to remain independent in the community. We work in partnership
with you to meet your changing needs and to maintain your independence.
Sometimes this may mean providing more support at times when you need it and at other times reducing support to ensure that you do not lose your independence.

How to access CHSP through KNC 

To receive services you will need to be assessed by My Aged Care, the assessor will visit your home to determine the appropriate level of support that is required to maintain your independence.

Support is provided based on your assessed needs. Referrals can be made by My Aged Care, GP, family members and friends. Self referrals are also welcome.

Clients are required to pay a client contribution, which will be advised at the time you register with KNC.
