PACE Early Intervention Program
Are you or a loved one worried about a recent diagnosis? Not sure of the next steps?
The PACE Early Intervention Program, funded by the Sydney North Health Network, aims to help you or a loved one who has recently been diagnosed with a chronic condition or are at risk of developing one.
The PACE program is available to provide 1:1 support and critical information about your diagnosis, symptoms and to connect you to the right services and groups that enable healthy ageing and management of chronic conditions.
A dedicated 'PACE Partner' will provide ongoing support to help navigate, guide and connect you with various providers or community groups (eg. home care, physio, emotional and social support) so you can form a partnership to help your journey.
The service is available Monday - Friday 9.00am – 5.00pm and will accept medical, nursing, allied health, and self-referrals.
Services offered through the PACE program:
- Needs assessment and plan
- Support & mentoring
- Warm referrals to providers, groups and services
- Allied Health service provision – up to 6-8 sessions and then review
- Connecting with social groups and community programs/activities
- Understanding and registering with My Aged Care
- Navigating services and supports available
